Vendor Performance Form

We want to hear from you whenever you are having difficult issues with a vendor.
You may not know that a performance form is available at:

Complete the form and report to DAS GSE - Procurement Services your issues on any vendor's performance or unsatisfactory products/ services. We will assist you in resolving the issues, if you wish. Just let us know. If you have good comments to share, we would like to hear those also!

We will keep a file, by vendor, of all such issues for the purpose of researching a vendor's past performance when considering future contracts, renewing contracts or making purchases.

That has to be of enormous value for any agency to know what performance issues other agencies may have had with a vendor or product.
